Job Overview/Summary:
We are seeking a highly motivated and detail-oriented Hospital Liaison (Clinical Liaison) to join our healthcare team. This role is responsible for fostering relationships with hospitals and community organizations to generate patient referrals and facilitate smooth admissions into our skilled nursing or rehabilitation facilities.
Responsibilities:

Establish and maintain relationships with local hospitals, physicians, and community organizations.

Drive consistent admissions to the facility by promoting services and managing referral sources.

Understand and manage the full admissions process, including policy, assessments, and documentation.

Represent the facility in a positive light through public relations and community engagement.

Stay updated on Medicare, Medicaid, and Managed Care reimbursement regulations.

Communicate the specific needs of new admissions to internal staff to ensure smooth transitions.

Coordinate and conduct facility presentations and on-site tours.

Obtain and maintain current insurance authorizations.

Review clinical referrals to ensure appropriate patient placement.

Travel between facilities as needed to generate and manage referrals.
Primary Skills:

Strong communication and relationship-building abilities

Knowledge of the admissions process in healthcare settings

Proficiency in managing clinical assessments and patient placement decisions

Familiarity with reimbursement processes (Medicare/Medicaid/Managed Care)

Qualifications:

2–3 years of experience in admissions at a SNF, subacute, rehab, or healthcare facility

Excellent organizational and documentation skills

Professional and effective telephone communication skills

Experience working with marketing databases and tools

LPN License preferred
