Qualifications:
Education:

Graduate of an accredited school of medicine (required)

Completion of an accredited Family Medicine residency program (required)

Minimum of three years practicing Family Medicine post-residency
Licensure/Certification/Registration:

Current MD or DO license or license-eligible in North Carolina

Board Certified in Family Medicine

Current DEA license (required)
