Job Title: Clinical Team Manager

Job Overview/Summary:

The Clinical Team Manager provides oversight and coordination of all patient care services and staff, ensuring effective and timely care delivery in accordance with regulatory standards. This leadership role involves strategic planning, clinical evaluation, personnel management, and quality control, all geared towards delivering high-quality home health care.

Responsibilities:

  • Oversee patient care services and staff assignments

  • Coordinate referrals and ensure continuous patient assessment

  • Plan, direct, and participate in delivery of home health services

  • Ensure timely initiation of patient care

  • Manage physician orders and communication among care teams

  • Approve and review individualized patient care plans

  • Integrate multidisciplinary physician orders into unified plans

  • Conduct regular OASIS reviews

  • Meet regularly with care team for coaching and updates

  • Initiate hiring, interview, and staff onboarding

  • Report performance metrics to Branch Director or Administrator

  • Be available for fieldwork as needed

  • Perform additional duties/projects as assigned

Primary Skills:

  • Leadership and team management in clinical settings

  • Home health care delivery and coordination

  • Clinical assessment and planning

  • Interdisciplinary team communication

  • Use of EHR systems (e.g., Homecare Homebase)

Secondary Skills (Good to Have):

  • Familiarity with Microsoft Office and Workday

  • Knowledge of hospice care practices

  • Strong coaching and mentoring abilities

Qualifications:

  • Associate degree in Nursing (required)

  • Minimum 2 years of RN experience in Home Health or Hospice

  • Minimum 1 year of supervisory experience

  • Valid, unrestricted RN license in the state of employment

  • Ability to travel locally with dependable vehicle, driver’s license, and insurance

  • Must be physically able to lift 50–100 lbs and perform job duties under high stress
