Job Overview/Summary:
We are seeking a compassionate and dedicated Home Health Nurse (RN) to provide in-home care and case management to elderly individuals and adults with disabilities. This role requires strong assessment, planning, and documentation skills, as well as the ability to work independently within a home care setting.
Responsibilities:
Assess client status during initial home visits to identify needs and develop comprehensive case management plans
Counsel and instruct patients, families, and caregivers in meeting nursing and related care needs
Complete and coordinate MDS-HC and other clinical assessments
Document accurately and maintain clinical records for each patient
Train, evaluate, and supervise SFC caregivers in collaboration with the care manager
Monitor the health status of members to ensure the proper delivery of SFC services
Primary Skills:
Active Registered Nurse (RN) license in the state of employment
Minimum 2 years of recent experience in direct care of elders or adults with disabilities
Strong case management, care planning, and assessment skills in a home care setting
Proficiency in Microsoft Office and case management systems
Excellent interpersonal, organizational, and communication skills
Secondary Skills (Good to Have):
Experience with Minimum Data Set (MDS) assessment tools
Familiarity with home and community-based services and supports
Qualifications:
Registered Nurse (RN) license – active and in good standing
Valid driver’s license and automobile insurance
Ability to travel within assigned geographic area
