Job Title: Home Health Nurse

Job Overview/Summary:

We are seeking a compassionate and dedicated Home Health Nurse (RN) to provide in-home care and case management to elderly individuals and adults with disabilities. This role requires strong assessment, planning, and documentation skills, as well as the ability to work independently within a home care setting.

Responsibilities:

  • Assess client status during initial home visits to identify needs and develop comprehensive case management plans

  • Counsel and instruct patients, families, and caregivers in meeting nursing and related care needs

  • Complete and coordinate MDS-HC and other clinical assessments

  • Document accurately and maintain clinical records for each patient

  • Train, evaluate, and supervise SFC caregivers in collaboration with the care manager

  • Monitor the health status of members to ensure the proper delivery of SFC services

Primary Skills:

  • Active Registered Nurse (RN) license in the state of employment

  • Minimum 2 years of recent experience in direct care of elders or adults with disabilities

  • Strong case management, care planning, and assessment skills in a home care setting

  • Proficiency in Microsoft Office and case management systems

  • Excellent interpersonal, organizational, and communication skills

Secondary Skills (Good to Have):

  • Experience with Minimum Data Set (MDS) assessment tools

  • Familiarity with home and community-based services and supports

Qualifications:

  • Registered Nurse (RN) license – active and in good standing

  • Valid driver’s license and automobile insurance

  • Ability to travel within assigned geographic area
