Job Overview/Summary:
We are seeking a Bone Marrow Transplant Clinical Pharmacy Specialist to join our healthcare team. This role involves delivering direct patient-centered care, supporting pharmacy operations, advancing medication safety, and serving as a clinical expert in bone marrow transplant care. The specialist will also contribute to teaching, mentoring, and quality improvement initiatives.
Key Responsibilities:

Provide direct patient-centered pharmaceutical care in collaboration with physicians, nurses, and other healthcare providers.

Facilitate pharmaceutical care services, patient care programs, and medication utilization aligned with evidence-based best practices.

Design and implement stewardship activities, restriction, and surveillance programs.

Serve as an active member of multidisciplinary teams, providing evidence-based medication recommendations.

Participate in medical emergency management.

Perform discharge medication review, reconciliation, and patient counseling.

Identify and resolve drug-related problems (e.g., drug selection, dosage, interactions, adverse reactions).

Lead clinical projects, quality improvement initiatives, and medication-use system enhancements.

Act as preceptor, developing student, resident, and staff training programs.

Facilitate procurement, ordering, and dispensing of specialty medications.

Ensure continuity of pharmaceutical care across shifts and staff.
Primary Requirements:

Graduate of an accredited pharmacy degree program.

Licensure as a Doctor of Pharmacy by the Oklahoma State Board of Pharmacy.

Eligible for preceptor certification by the Oklahoma State Board of Pharmacy.

PGY-1 Pharmacy Residency (preferred) or minimum 3 years of clinical pharmacist experience.
Preferred Qualifications:

Board Certified Pharmacotherapy Specialist (BCPS).

BLS certification and Immunization Administration Certificate (preferred; BLS required if Immunization Administration Certification is held).

Previous experience in bone marrow transplant or hematology/oncology pharmacy practice.
Knowledge, Skills, and Abilities:

Strong knowledge of hospital/clinical practice standards and regulations.

Expertise in specialty medication therapy management and advanced pharmacy practice.

Excellent verbal and written communication skills for interdisciplinary collaboration.

Ability to lead teaching, preceptorship, and training initiatives.

Proficiency with Microsoft Office and Electronic Health Record (EHR) tools.
