Job Overview/Summary:

The Ambulatory RN or LPN is responsible for delivering high-quality nursing care within an outpatient clinical setting. This role focuses on telephone triage, patient education, medication management, quality initiatives, and clinical support, ensuring efficient and patient-centered care in alignment with established practice guidelines.

Responsibilities:

Perform telephone triage according to established practice guidelines

Provide comprehensive patient education and follow-up support

Process and manage medication refills accurately and efficiently

Participate in and support quality improvement initiatives

Administer injections and other clinical procedures as necessary

Document patient interactions, conversations, and actions taken accurately in the EMR

Assist in developing and implementing applicable policies and procedures

Primary Skills:

Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Illinois

Strong telephone triage skills

Excellent oral and written communication skills

Strong organizational and prioritization abilities

Ability to accurately document patient care and clinical decisions

American Heart Association CPR certification required

Secondary Skills (Good to Have):

Experience with medical practice management systems

Experience with EMR software, specifically Epic

Proficiency in Microsoft Office Suite, particularly Word and Excel

Qualifications:

Current State of Illinois RN or LPN license required

Associate’s Degree in Nursing required

Bachelor’s Degree in Nursing (BSN) preferred; BSN must be achieved within 5 years of hire (if applicable)

CPR certification from the American Heart Association required
