Job Title: Ambulatory Clinical Pharmacist

Job Overview/Summary:

The Ambulatory Clinical Pharmacist plays a vital role in delivering safe, effective, and evidence-based pharmaceutical care to patients in ambulatory and hospital settings. This position involves reviewing provider orders, dispensing medications, maintaining accurate patient medication profiles, and collaborating with the healthcare team to optimize therapeutic outcomes. The pharmacist will oversee pharmacy technicians and students, ensuring accuracy, compliance, and adherence to pharmacy standards and policies.

Responsibilities:

  • Review and verify provider orders to ensure appropriate medication selection, dosing, and administration.

  • Dispense medications and pharmaceutical preparations to inpatient and outpatient areas.

  • Maintain and monitor accurate patient medication profiles to promote optimal therapy management.

  • Provide drug therapy consultations and recommendations to physicians and healthcare staff.

  • Supervise pharmacy technicians and students, verifying the accuracy and completeness of their work.

  • Ensure compliance with all Pharmacy Policies, Procedures, and regulatory standards.

  • Participate in interdisciplinary patient care rounds and provide clinical input when necessary.

  • Support the implementation of pharmacy programs aimed at improving patient outcomes.

  • Travel to and staff all hospital pharmacy sites based on operational needs.

Primary Skills:

  • Strong knowledge of pharmacology, therapeutics, and clinical pharmacy practices.

  • Proficiency in medication order verification and patient profile management.

  • Excellent communication and collaboration skills with healthcare professionals.

  • Attention to detail and commitment to accuracy in dispensing and documentation.

  • Leadership abilities to supervise and mentor technicians and students.

Secondary Skills (Good to Have):

  • Experience in an ambulatory care or hospital pharmacy setting.

  • Familiarity with electronic medical records (EMR) and pharmacy management systems.

  • Knowledge of state and federal pharmacy regulations.

Qualifications:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited College of Pharmacy (required).

  • Active Pennsylvania Pharmacist License (required).

  • Previous experience in hospital or ambulatory care settings preferred.
